Pport - expiry date?

I'm 16 years old and going on holiday from June-Aug. . My pport is going to expire on September this year (2009) and my travel agents said that i should make a new pport because there needs to be atleast 6 months validation in your pport when your going on holiday. Where do I get the application form to renew my pport and since I'm going to be 17 in September this year, do i need to apply for an adult or child pport?
